New Teamster Health Insurance Cards

The New Teamster Health Insurance Cards replace your current Medical, Dental, and Prescription Drug cards. Please note that your benefits have not changed.

Please discard your old cards and replace them with the new card. Present the new card to any of your service providers and ask them to update their records.

We also would like to remind you that you may want to update your Life Insurance Beneficiary designation. You can obtain a beneficiary designation form by contacting Teamsters Local 404 H.S.I.P. at 413-733-2191.
